I wrote several times in the past that I feel my peripheral neuropathy has gotten worse, but I was a bit uncertain each time. However, 11-months post-transplant, I know it is worse than a year ago. If I had to guess, I think it is due to having higher blood sugar from having a working kidney and some of the medications I am taking.
For example, right now I have three areas of pain in my feet. They are not as intense as a normal attack, but each is a low-level background pain. I have never had three areas of my feet hurting at once. The overall numbness has increased as well. I can barely move my toes on my right foot, and the tingling sensation is occasionally preventing me from sleeping. Finally, the frequency of intense pain attacks have decreased from about monthly to multiple times per week. Of course, this is still better than being on dialysis, but the numbness and pain is really stressing me out.
This is WHO data for the entire world. There are clear spikes from December 2020, April 2021, September 2021, and right now. For the US, the data is a bit different, but cases are also going up again.
Finally, data from Orange Conty. It does look like cases are picking up here too. I do not know if that is because of the Omicron variant, or just the consequence of everyone meeting up for Thanksgiving.
For me, I still have not left the house that much. Other than doctor appointments, I am still only traveling to my sister’s house, the local Albertsons, and meeting some friends for lunch last Friday. Our department is still having a holiday party at a rented house in Hermosa Beach, but I have already declined the invitation. Likewise, our church cell group is having dinner at a local Korean shabu-shabu restaurant. I think the place will be super crowded, and everyone eating out of one pot is not a good option for me at this time.
The FDA did announce approving an antibody shot for immuno-compromised people. I think I will make a new post for that.

Oops, forgot why I was posting about this topic in the first place. I have been seeing a lot of news about continued protests regarding vaccine mandates. With more people testing positive and death rates not going down, I really cannot understand these idiots. I also read that these anti-vaxxers want to get antibody shots if they get sick. Whatever happened to not putting anything “untested” into their bodies?
I finally got a kidney ultrasound two days ago. UCLA Transplant Center told me early on that I would need to get periodic kidney ultrasounds to check on the existing cyst in the donated kidney. This cyst was what held up my transplant for over three years.
I was scheduled to go in at 4:00 pm. By the time I checked in, the outpatient clinic building was deserted. The entire procedure took about 45 minutes, so I was out and home by 5:15 pm. Initially, there were some questions whether we were scanning the OG kidneys or the transplanted kidney. In looking up my records, the ultrasound technician mentioned that the last scan was five years ago, and she was the technician then as well; she has worked at the same clinic for 25 years. I have worked about 26 years since my last graduate degree, but it was spread over three (or four) companies.
My nephrologist called me up the next day and said the original cyst was fine. There were some other small cysts but nothing important. I am not sure if those were there before the transplant. There was also something that was a bit enlarged, but also nothing to worry about at this point. She will send the ultrasound to UCLA next week so they can evaluate the results as well.
I have been getting non-stop advertisements in Instagram for these special caps for insulin pens. The story goes that if your insulin pen heats up beyond a certain temperature, then the insulin left in the pen becomes damaged and ineffective. It felt like each time I opened the Instagram app, I would see an ad for Vivi Caps.
The price for each cap is $110, though you get a discount online if you buy more than one. I ended up buying one for my Humalog pen from Amazon for $80. A quick Google search did not turn up any negative reviews of the product, but it did seem like there were lots of “paid” positive reviews.
So why did I buy this fancy cap? I do not know if I am even experiencing the issue that the cap is trying to solve. There is indicator on the pen letting the patient know if the remaining insulin is problematic. I do take the pen with me if I am eating away from home, but there is no indication if the product is working or not. There is just a nagging thought that my blood sugar levels are not decreasing enough after insulin shots.
So, is this product real, or an elaborate scam? There is no indicator other than a battery check LED. Since the goal of the cap is to maintain the pen at a cool temperature, there should be some indicators to let the user know the operating conditions. Again, I would have no idea if the product is doing anything.
After getting some not-so-great-tasting Evolve protein shakes from Costco, I decided to try another brand. So far, I have tried Nepro (for dialysis patients), Ensure, Premier Protein, Muscle Milk, and of course Evolve. Taste-wise, the best so far was Ensure and Premier Protein. Since I am running low(ish) on the Evolve protein shakes, I checked out Costco and Amazon again for other options. This time, the least expensive option was Atkins. With only seven grams of carbs and no added sugar, these seemed like an okay choice.
I bought twelve bottles of chocolate and twelve bottles of strawberry from Amazon. The price was about $16 for each 12-pack so ~$1.33 each. I just tried the strawberry this morning, and it tasted worse than the Nepro I drank while on dialysis. There is a heavy medicine taste. I am unsure why that is the case. Is there a protein source that tastes terrible but must be added to all these shakes?
I also finally looked online for some reviews. It seems that nutritionists like the plant-based protein shakes. Evolve was not on any lists but many had OWYN brand. These shakes are expensive, selling for ~$3.50/bottle on their own website for the Pro Elite version. Costco does have the same product for $1.67/bottle (18 for $30) so maybe I will try this next. Hopefully, it does not have the same weird veggie taste.

The chart only shows eight months of data, but it has the recent delta spike. From the chart, the number of new cases is steady at ~20 per 100k people per day. That does not seem like a lot, but Orange County has 3.2 million people so this represents about 640 cases per day. If we look at the entire country, there are still ~1,000 people dying of COVID daily.
I went to Panini Kabob Grill in Santa Ana to pick up dinner last night. There was a line of people outside waiting for tables, and it was mostly full inside the restaurant. Other than me and the staff, no one had a mask on, even though multiple parties were standing right next to each other in line. Do people think we are back to normal now?
Looking at the numbers from last year, I am expecting another spike for Thanksgiving/Christmas/New Year’s Day. It probably will not be as bad since the vaccine was not available last year, but 1/3 of the population are still not vaccinated. I guess we will wait and sew what happens. If the case rate remains flat, then I would feel a lot better about going back to work in-person.
My gaming PC is working again. I spent about $165 total on a new power supply, a new hard drive, and a SATA adapter. The computer is better than it was before since the power supply outputs 650 watts vs. 460 watts. That means the power supply runs cooler; it also has a much larger fan. My 3.5″ drive is fairly old, coming from a previous PC. It also runs extremely hot. The 2.5″ drive is much cooler and quieter. Overall, the PC now runs cooler, and I can tell the fan speeds are lower too.
This is the drive I purchased. Interestingly, I bought the same drive four years ago when I first got the PC. There was a tiny 256GB SSD included, and I added the (now broken) 2TB drive and purchased the smaller 2TB drive to hold games. Hopefully after the latest fix, the computer will last a few more years.
I read on Reddit that each year of dialysis shortens your lifespan by five years. I am fairly sure someone just made that up, but I tried googling anyway. There was nothing that substantiated that claim, but I did find this paper from 2010.
The expected mean survival of a 55-year-old American is 26 years. According to the United States Renal Data System (USRDS) 2009 report, the expected survival for a 55-year-old person with a kidney transplant is 15 years, but the expected survival of a 55-year-old person on dialysis is only 5 years.
I am 53 years old. Not counting the three and a half years I was on dialysis, if we add 15 years, then I can expect to live to sixty-eight. If I were still on dialysis and waiting for a deceased donor kidney, chances are I would be dead before receiving a transplant. Sobering.
The most important question for me now is when I should retire. Since the study was more about frequent versus conventional dialysis, there is not much more detail on post-transplant life expectancy. If we use seventy-five as a stretch goal, I have 22 more years to live. A while back, I estimated that I can live comfortably on $50k-$60k per year. I do not have any debt: house, car, and student loans are all paid-off, and I pay my credit card balance in full every month. If we use $60k, then that is about $1.3M without inflation. If we assume a 2% inflation, then the total is $1.6M. At 5% inflation, the total I will need for 22-years is $2.3M.
Finally, the elephant in the room is health insurance. If I retire before sixty-something, I will need to purchase my own health insurance. Pretty sure that will be a lot with all my health issues. No idea what to do about that yet. Move to Canada?
After sitting in the freezer for a few weeks, I finally microwaved it and ate it for lunch yesterday. The taste was surprisingly decent. I grew up liking the canned Dinty Moore beef stew, even though it was probably unhealthy to eat. The cooking/heating instructions worked well and there was enough for a small lunch. However, once again, the marketing and packaging photos lie. Here is a photo from the website:
First, the bowl was really shallow. There is probably less than 1/3 of the food amount implied by the photo above. Secondly, I could count 7-8 pieces of beef. That makes sense since this was a “beef” bowl. Imagine my disappointment when in real life, there was only two pieces of beef. Everything else that looked like meat was actually a piece of potato. Again, the taste was fine, but I did not know I was buying a vegetable stew.
It is even worse if you look at the back of the box. You cannot say “meat matters” when there is barely any meat in your beef stew. Shame!
Having said that, I would buy again if the entrée was on sale. I am too lazy to make a table for nutritional information today; it is available online. Basically it has too much salt, but the rest is fine.
My gaming PC is dead. I purchased the system directly from Dell back on 11/8/2017 so the computer lasted four years. At the time, I had some spare parts leftover from another dead PC, so I purchased a bare-boned system from Dell. I then added more memory, a second SSD, a large internal HDD, and replaced the graphics card with my older Nvidia GT970. There were son initial issues with power management, but the system has worked well for the past four years. Today, there was a notification to reboot the computer to complete some random software update. After the install, my PC turned off and just sat there. I tried turning it on several times since, but the PC just sits there like a large brick.
After much googling, and the fact that the self-test on the power supply unit (PSU) is negative, I purchased a replacement from Amazon. It was a 650W EVGA PSU that was highly recommended by many sources. The current dead PSU is only rates at 460W. The unit cost $90 and will arrive tomorrow.
I really hope this works. It will already be a huge pain-in-the-ass to swap out the PSU. There appears to be a myriad of connectors to remove and reconnect.
One of the medications I take is Atovaquone Oral Suspension. I believe it was a replacement for Bactrim, and I have been taking 10 mL each day since med-February. My nurse-practitioner said some patients liked the taste; I have always hated it. For the most recent refill, the medication came from a different manufacturer. Unbelievably, it now tastes even worse than before. The previous medication was made by Glenmark Pharmaceuticals Limited in Mahwah, NJ; the current maker is Camber Pharmaceuticals, Inc. in Piscataway, NJ. Weird that both are in New Jersey.
Looking more closely, it appears that neither company made the medication. Both products were made in India, one in Himachal Pradesh, and the other in Hyderabad. This is a liquid medication so high on the weight vs. value scale. I have always heard that India was a huge manufacturer of generic medication so it makes sense they would also contract manufacture for American pharmaceutical companies.
I am just upset that the new product tastes more horrible then the first.

I just realized that one of the side effects of Atovaquone is “difficulty falling asleep of staying asleep.” Since you are supposed to not take other medication three hours before/after the Atovaquone, I have been waiting until the late afternoon to take the medication. I should try to take it as early as possible to see if that will help with my drug-induced insomnia.
Well, the shit has certainly hit the fan compared to three months ago. Back then, I make a post about how we were getting conflicting signals at work about the LA County mask mandate. How quickly things change. Since my company is considered a federal contractor, we are subject to Biden’s executive order 14042. That means everyone at work needs to be fully vaccinated, or get a medical or religious exemption.
Last week, our work put out a long email detailing the vaccination requirements, and instructions on how to get an exemption if qualified. If an employee does not follow the rules, they can be subject to termination. I hope the requirements for getting an exemption is fairly stringent, else every anti-vax idiot will just lie and get an exemption. I would be very disappointed if any of my department co-workers are in the anti-vax group without a valid medical/religious reason. Hopefully, with there will be very few exemptions and once everyone else is vaccinated, I can go back to the office a few days each week to work.

Speaking of the federal vaccination mandate, we have a new member in my church cell group. I think he has been at the church for many years but just switching cell groups. Anyway, he is currently a federal employee and is adamantly opposed to getting the COVID vaccine. I assume he is vaccinated for everything else like MMR, diphtheria, tetanus, etc. He was seriously contemplating getting fired and losing pension, to moving to a more vaccination-lax state, to lying about needing a religious exemption. He also seems to be a MAGA kind of guy. I really do not understand the mentality. It does appear that people are incapable of admitting error anymore. Maybe there was some uncertainty when the vaccines were just released, but now billions of people have received the various COVID vaccines, and it has probably saved millions of lives.
I had a large stash of Premier Protein shakes in various flavors: chocolate, café latte, and strawberries & cream. They tasted fine and I was drinking almost one per day for a quick breakfast. The only worrisome item was the high calcium content (650 mg or 50% DV) since my calcium level were borderline high. Well, my supply ran out last week so I was looking to order some more. I am pretty agnostic on the actual brand, and more concerned with the nutritional content and price.
I checked my local supermarket, Amazon, and Costco, and there was not anything on sale. Most shakes were selling for about $2 per bottle. I finally came across this at costco.com. Right now, it is $27.49 for 18 bottles, but it was only $21.99 at the time so I bought two boxes. I was a bit concerned that I have never tasted the brand before but most shakes taste about the same in the past.
I received the shipment via UPS yesterday and I already drank two bottles. The flavor is definitely not similar to the Premier Protein or Ensure. Evolve is advertised as a plant-based protein shake, and I think I can taste the difference. It is not my favorite, but that the price of being cheap. I think I will go back to Premier Protein next time it is on sale at Costco. First I have to finish all 36 bottles of the veggie shake.

Some minor differences. Is that due to the “plant-based” protein? What is the source of other protein shakes if not plants? Animals? Also, Premier Protein website has a huge list of other minerals and vitamins in their shakes whereas there appears to be no other information for Evolve on their website. The nutritional information listed is the exact same as found on the actual shake packaging. Does that mean Evolve does not have additional nutrients?
Wow, time flies. I have not updated one of these posts since right after the kidney transplant. Since today is nine months post-transplant, I thought I would post an update. Here we go!
ESRD: Things are still pretty good. My creatinine levels have gone up and down over the past nine month, and the last test came back at 1.34. Both my nephrologist and the transplant center are not concerned.
Dialysis access: Fistula is still live and making noise. It is definitely less noticeable, but the bumps at each cannulation site are not longer there.
Cardiac rehab: I am doing my own walking and bought some light free weights. Have not heard from the rehab center since mid-last year.
Peripheral neuropathy: Ironically, I am in the middle of an attack right now. It started after dinner last night and has kept me up until now. I thought it was getting better at one point, but the pain level is increasing again. Probably a six on the pain scale currently.
Back pain and pressure in chest: Back is okay now. That was likely an effect of the CABG surgery. I still have some minor pressure in my chest if I sleep to my left, but not really bothersome.
Minimal urine production: All good. Peeing a lot each day.
Insomnia: It was better for awhile but it is pretty bad again. My highly variable workload is not helping. Still taking lots of naps during the day and not sleeping well at night. It is 3:00 am right now.
Dental issues: Sigh. A few more new and replacement crowns. I think I am pretty much done with major dental work, but need to decide whether I want a dental implant for one of my lower molars.
Endocrinologist: Seen her a few times. Blood sugar is still not controlled so insulin amounts have been steadily increasing for each shot.
Lump in left leg: Mostly gone. There is just a very small bump and barely noticeable.
Constipation: No problem. Pooping every day almost.
Ophthalmologist: Finally went and visited a retina specialist. After two appointments, there is nothing to treat but need bi-annual appointments. Regular ophthalmologist is still good with annual visits.
Surgery recovery: Everything seems fine. Walking pretty well after surgery until now. There is occasional abdominal pain but not really near the transplanted kidney so doctors not worried.
COVID-19: Got three Moderna vaccine shots but have not tested for antibodies. Still working from home and not venturing out that much. Basically been out of the house only once during the last two weeks; had to go to Albertsons to get some diet soda.
Sore leg: That was purely physical pain from stupid fall at UCLA. Leg hurt for longer than I thought but no issues now.
Not really a health issue but my podiatrist wants me to get fitted for new shoes. I did this many years ago and have been buying the same New Balance cross-trainers on Amazon. Probably need new shoe liners.
I think that is it. There are some other issues being watched due to test results like high hemoglobin and potassium. If I do any more of these updates, I will remove some of the items that are not applicable anymore.
Boston Market was a restaurant chain that offered semi-fast food service where you can order meals with carved meats and sides. They were pretty popular at one point, but I am unsure if there are still any stores remaining. Hmm, there appears to be 344 stores remaining across America. The ones that were close to where I live are closed though.
On my last trip to the supermarket, I found these Boston Market frozen entrees. The bowls were two for $5 so I bought two to try. I had the Fried Chicken Bowl just now for lunch. I was too lazy to type out the nutritional information so I just took a screenshot:
I bought this meal because the nutritional content was not too bad. There is a bit too much sodium, but that is typically true for all commercially prepped food. The carbohydrate and sugar content was pretty low so I am hoping that it will not spike my blood sugar. Since there is quite a bit of potatoes, I was concerned about the potassium content. Unfortunately, that information is not listed on the package. As for taste, it is about a 3 out of 5. Because the instructions said to mix the ingredients during microwaving, all the food flavors were mixed together. There was bacon in the mashed potatoes and it overwhelmed the taste of just about everything else. The chicken was okay, but you could not tell that it was “fried” due to microwave cooking. If the price is still $2.50, I would probably buy it again.

Definitely not the same. It is also a bit overcooked, even though I took 30 seconds off the cooking time. The bowl was pretty shallow so you are not getting a huge bowl of mashed potatoes implied by the website photo. There was a lot of chicken though. Typically companies skimp on the meat protein part of the meal, but this was pretty filling. I did buy another different Boston Market frozen enteée so I will have to try it later.

As I mentioned previously, it is like having a second job managing post-transplant tests, doctor appointments, and medications. After the near-fiasco refill for Tacrolimus last time, I asked my nephrologist to send in the next refill prescription early on October 18.
Since then, I have been checking the pharmacy app. Since Accredo (specialty pharmacy) is part of Express Scripts, there always seems to be some overlap between the two apps. For the past week, I kept seeing the refill as in-progress at Express Scripts, but no on the Accredo app. Today, I finally went to their website on my PC and saw that the order was processed but has a delivery date of November 11.
Everything is good, right? Nope. I received the last order on August 11. I know it does not take three weeks for a refill so maybe they are waiting three months? However, the prescription is for a 90-day supply, and there are 92 days between August 11 and November 11. If we follow that logic, then I will always short by 5-6 days each year. That is not good for something like an anti-rejection medication like Tacrolimus.
Anyway, I called the pharmacy and their system said the refill can be delivered in two days. That is totally great and all, but I wonder what would happen if I did not call them myself? Would the medication still come on Wednesday (unlikely), or will it show up on November 11 when I only have enough on-hand until November 9? On the positive side, the 90-day refill is only $10.
During the past three weeks, I had three separate lab tests. The first was for my regular nephrologist, the second was for my endocrinologist, and the third was for the UCLA Transplant Center. All the appointments were scheduled a week from each other. I tried moving the schedule to reduce the number of labs, but was unsuccessful. The first two were done at my local medical group’s labs in Orange, while the tests for UCLA were their Laguna Hills facility.

I do not know if inconsistent is the right word when I do not know the accuracy of the tests. If the results are plus/minus 20%, then maybe the results are consistent within the error bars. My regular nephrologist did up my prescription for Losartan, which can lower hemoglobin but also lower blood pressure and increase potassium. Maybe that did have an effect since my potassium results do trend higher and blood pressure has been lower in the past few weeks.
My endocrinologist did ask for a bunch of other tests, mostly A1C/Fructosamine and a lipid panel. My cholesterol was fine, but from the test results and the readings journal from my glucose meter, she added more insulin: 28 units of Trujeo and 8/8/10 plus sliding scale for Humalog before meals. Good thing I have a large supply of insulin pens in my refrigerator.
Of course, four insulin shots daily means a lot of disposed pen needles. I filled up my first two gallon sharp container and just purchased two more from Amazon. Now I need to find a nearby hazardous waste disposal center.
UCHealth Denies Kidney Transplant To Unvaccinated Woman & Donor
AURORA, Colo. (CBS4) – A Colorado woman with stage 5 renal failure was months away from getting a new kidney. Now, she and her donor are looking for another hospital after learning UCHealth’s new policy.
According to UCHealth, the majority of transplant recipients and living donors are now required to be vaccinated against COVID-19. Neither woman has received their shots.
So when my transplant was scheduled in late January, I was not yet eligible for a COVID shot. Unlike the two women in the article, I was super worried about getting COVID in the hospital and losing the kidney after waiting 4+ years. As I posted before, UCLA was basically out of ICU beds, and I had to share recovery with another patient, both of us crammed into a room designed for one bed.
Further down, the article says:
“It’s your choice on what treatment you have. In Leilani’s case, the choice has been taken from her. Her life has now been held hostage because of this mandate,” said Fougner.
Fougner says she hasn’t received the vaccine for religious reasons. Lutali hasn’t gotten the shot because she says there are too many unknowns. Until last week, neither woman thought they needed to be vaccinated for the transplant.
You can tell they are anti-vax idiots by their hyperbole. Really, “held hostage!?” Nobody is forcing them to do anything. They are free to find another transplant center (good luck!) that will do the procedure without requiring vaccinations. Both the reason they give is totally bullshit as well. What religion forbids a mRNA vaccine yet allows for a complex surgical procedure with lots and lots of different drugs. Is she not going to take anti-rejection medication post transplant? Those have more side-effects than any COVID vaccine. The donor is also stupid with the “too many unknowns” crap. There are hundreds of things that can go wrong with both donor and recipient post-transplant. I am pretty sure they had to follow a bunch of appointments and test to get to this point in the process; for sure me and my sister did. Are they going to reject what their doctors and surgeons are telling them at the very last step? Ugh.
Another thought. What if the recipient is willing to get a vaccine? Will the donor say “fuck you” and not go through with the donation if she needs to be vaccinated?
“Here I am, willing to be a direct donor to her. It does not affect any other patient on the transplant list,” said Fougner. “How can I sit here and allow them to murder my friend when I’ve got a perfectly good kidney and can save her life?”
The women haven’t been able to find a hospital in Colorado that will do the transplant while they’re unvaccinated. They’re now looking at other states.
More hyperbole. It is not murder. If the transplant recipient values anti-vax nonsense over her own life, then so be it. My prediction? They will come crawling back to the initial transplant center as they will not fine anyone else to do the procedure.
I normally take both insulin shots in the morning. One is a long-lasting insulin for the entire day, and the other is a fast-acting insulin taken before meals to mitigate the blood sugar spike after eating.
This morning, the blood sugar reading was 149, so according to the sliding scale from my endocrinologist, I injected eight units of the fast-acting insulin. You are supposed to eat afterwards but I got distracted by some work texts and emails so I did not eat. After about 15-20 minutes, I began to feel warm and dizzy. Having experienced this several times before, I knew my blood sugar was likely too low due to the insulin shot. How low? It was at 80 when I measured it.
Guideline says normal range is 70 to 130, but for me, anything below 90 and I start feeling weird. I quickly took one glucose pill, and ate some crackers. I feel a bit better now, but will probably take a short nap soon since I do not have any work meetings this morning. I can see how insulin OD can be very dangerous.

Weird. So after the low blood sugar reading, I took one glucose tablet, a protein shake with very low carbs and almost no sugar, half a biscotti biscuit, and about 6 saltine crackers. I just (2:00 pm) measured my blood sugar again, twice, and got 279 and 249. I do not understand why the number jumped that much, even after five hours. So I had to shoot another 10 units of insulin and hope my salad croutons has enough carbs so I will not get low sugar again.
Ever since I bought the Apple Watch, I have been using it to track workouts instead of a Fitbit. Every month, there is an award you can win on the iOS Fitness app if you beat some challenge, . I have been slacking in exercising but I have managed to pick up one award every three months… exactly.
That is March, June, and September. I think there are a few more too. I am not bragging since the challenges were trivial. September was just to do five workouts. The March one was harder since it was exercising > 43.7 miles. Such a randomly precise number.
I am going to try to walk on the treadmill every night. I downloaded some anime series and found that the time passes much faster if I am watching something interesting.
